Change Your Name on Your Voter Registration

If you register to vote and subsequently change your legal name, you will need to re-register with the new name. If you have a Utah Driver License or State ID with your new name and current address you can register online. Or, if you prefer, you can fill out a new voter registration form and submit it to us via email or US Postal Mail.

Option 1: Re-Register Online

You must have a current Utah Driver License or State ID with your new name. If you do, CLICK HERE to re-register online. 

Option 2: Mail or In-Person Registration

Download and fill out the voter registration form and return it to us in person, by email, or via postal mail. All registration forms require a physical signature. We cannot legally accept electronic signatures for voter registration. There are also paper copies available at the Clerk/Auditor's Office window at the address below.
